The Website Administrator also processes the following personal data on the Website for the following purposes:

Purpose: Transmission of user requests to Service Providers or Nurses

  • Personal data processed: name, surname, address of the place of residence, date of birth, height, weight, gender, information about ailments, diseases, identified need for permanent care, need for care (assistance), diagnoses, diseases, other health disorders, needs, allergies, sources of financing of the service, as well as name, telephone number, e-mail address of the contact person, family relations with the person who needs services.
  • Legal basis: consent.
  • Data transfer: The data provided is transferred to the Service Provider(s) or Nurse(s) of your choice.
  • Please note: Provide only as much data as you agree to provide to the Service Provider or Nurse and as much as you have the right to provide, representing the person who needs the services. Before providing another person’s data, in particular health data, you must ensure that you have that person’s valid consent to provide their data, including health data, to Service Providers or Caregivers through the Site, or that you have a legal or other legal basis to represent such person.
  • Storage time: The Website Administrator stores the data provided in the request only for as long as is technically necessary for the transfer of data to the Service Provider or Nurse. If you have an Account on the Website, the data will be stored for one month so that you can use it to make new requests to Service Providers or Nurses.

Purpose: Provision of website services to Service Providers

  • Personal data processed: Name, surname, place of provision of services, telephone number, e-mail address, website address of the Service Provider (natural person) or representative of the Service Provider (legal entity).
  • Legal basis: contract (when processing the data of the Service Provider – a natural person) or a legitimate interest to carry out the activities of the Website administrator (when processing the personal data of the Service Provider – a representative of a legal entity).
  • The data provided by the Service Provider is made public on the Website.
  • Retention period: If a written contract is concluded, the data contained in the contract is stored for 10 years after the expiry of the contract. The Service Provider’s data is published on the Website for 5 years after the last update of the Content, unless the Website Administrator and the Service Provider agree on a different term.

Purpose: Provision of website services to Nurses

  • Personal data processed: Nurse’s name, surname (if specified by the Nurse), qualifications, advantages, experience, street and city of the address, photo, phone number, e-mail address.
  • Legal basis: contract.
  • The data provided by the nurse is published publicly on the Website.
  • Retention period: If a written contract is concluded, the data contained in the contract is stored for 10 years after the expiry of the contract. The Nurse’s data is published on the Website for 5 years after the last update of the Content, unless the Website Administrator and the Nurse agree on a different term.

Purpose: Creation and administration of accounts on the Website

  • Personal data processed: Account holder’s name, surname, account name and password, phone number, e-mail address.
  • Legal basis: contract.
  • Retention period: 2 years after the date of the last login to the Account.

Purpose: Administration and publicity of reviews

  • Personal data processed: name, surname (if indicated by the person), other data that the person indicates when writing a review, as well as e-mail address.
  • Legal basis: consent.
  • Retention period: 5 years after the post of the review.
  • The content of the review, including the name, surname (if indicated by the person), is published publicly on the Website. The email address is not published and is only used to protect reviews from unwanted content.

Purpose: Communication with Website Visitors

  • Personal data processed: name, surname, telephone number, e-mail address.
  • Legal basis: the desire to take action at the request of the data subject prior to the conclusion of the contract or the legitimate interest in carrying out the activity and/or ensuring quality.
  • Retention period: 1 year after the date of resolution of the request.

Objective: Provision of news and offers

  • Legal basis: consent.
  • Shelf life: 2 years after receipt of consent.
